Parental Consent Form (Social Media)

A parental consent form for parents to sign giving Pulse permission to use both photographs and video imagery of their children on all of Pulse's social medias, YouTube accounts and website.

At PULSE MUSIC, we love to celebrate the talent and growth of our young musicians! From time to time, we share photos and videos from lessons, rehearsals, performances, and events on our official social media platforms (such as Instagram, Facebook, and YouTube), as well as on our website. To ensure that we have your permission, please review and sign the consent form below.

My child’s last name will not be shared publicly unless I have given explicit additional permission. • These images and videos may be viewed by the public and could be reshared by others. • I can withdraw my consent at any time in writing, and PULSE MUSIC will cease future use (previous content may remain posted unless otherwise requested).
